Building Access Request Information
Please specify which doors or areas you need to access.
If you don't know the exact door numbers, be descriptive
(i.e. "Prof. Smith's labs", or "Physics 127B lab").
Do not assume we are aware of every location to which you might need access.
Exterior door codes for after-hours access are a separate item
from office/lab codes and must be specifically requested.
All requests for door codes or keys will be confirmed
with your supervisor or other department staff.
For your code, please select a secure 6-digit numeric value that you can remember.
Any door code found to be a geometric pattern, too simplistic, a physical or
mathematical constant, or in some other way easily associated with you
(i.e., your phone number) will be rejected. Codes such as: 123456, 789654,
159753, 314159, 112233 are requested every quarter, and rejected.
Door codes will be disabled when the user is no longer affiliated with
the UCSB Physics Department (i.e., graduation, employment separation).
Do not:
fill in this form for another individual.
share your door code with anyone. If an incident were
to take place in a specific room or lab, door
codes will be used to determine who was in the room.
allow any unauthorized person follow you into the room
if you believe they do not belong there.
