Physics Computer Services (PCS) Account Application form

Is this the form I should use?

This form is used to request access to a variety of computer and network related systems. Please be sure to provide all the information requested below to allow us to process your request as quickly as possible. We need to know how you are related to the Physics Department to provide the appropriate access. If your relationship to the department and its personnel cannot be deteremined, no access can be granted.

In general, this form is used by Faculty, Staff, Graduate Students and Undergraduate Students who work in the Physics Department. Individuals who work for iTST, CNSI, KITP or MRL should contact their IT staff to obtain appropriate accounts there. We do not provide accounts on their computer systems.

Note that incoming (Fall) Physics Grads should submit the Account Application for Incoming Graduate Students instead, as it requests information specific to incoming grads, and copies pertinent information to the Graduate Student Advisor, relating to Fall orientation.

Also note that requests for Physics VoIP phones use this form instead.

Building Access Requests are handled by a separate form. If you require keys or doorcodes please use this form.

To meet University Policy requirements, Undergraduate Students must have Supervisor's permission. They should submit this form via the web, then print the form and have it signed by their supervisor or class instructor. The form must be received by Physics Computing Services, signed, before we can process the account request.

NOTE: Accounts will be removed when the user ceases affiliation with the UCSB Physics Department (i.e. graduation, termination of employment). Be sure to fill out the separation forms provided to you by your department human resources person when you leave the department. That information lets PCS know what to do with your files, accounts and e-mail after you leave.

Computer Account Info

Username (no longer than 8 lowercase characters):
Note: The username will also be the user's email address, if requesting an e-mail address. username@physics.ucsb.edu

Temporary Password (6 to 8 char.) :
DO NOT enter a word, or anything related to your name or username.
This will be considered too simplistic, and we will assign a random password.
75% of the time, people pick a password that is too simplistic, and the systems reject it.
You will then be asked to call us or come by the office to obtain your password.
We DO NOT e-mail passwords.

Best idea: include numbers and symbols in your password.

 

Here we ask what services you are needing. If you don't know you need a service, specify "no".

Note also that each of these services has its own account database. Having an account on one service does not allow access to another.

Specify file server(s) you will access :
(i.e. Administrative Domain, Instructional Domain, CPOS Domain, SPINS Domain, Grad storage, or list researcher's host computer)
If none, then leave blank. If you don't know, provide the Principal Investigator's name, and we will guess the correct machine/domain.

Does the user need an email account in Physics ?
This provides you a place to receive and store e-mail.

If yes, select domain for email address :

Does the user need a Physics web server account?
This allows the user to have local webspace to host information about their research.

Does the user need a Physics VPN server account?
This provides the ability to connect into the department remotely to use services not accessible from outside the building.
Note that staff (which includes Postdocs and researchers) and faculty can use their UCSBNetID to access the VPN server, and do not need another account for that purpose. PLEASE do NOT request one.
Does the user need a Physics Wireless account?
This is for individuals that have registered a wireless computer to connect to the Broida network. If you haven't already registered your personal computer with PCS, PLEASE do not request a wireless account. If you don't work on the 5th or 6th floors of Broida, or in trailer 939 or trailer 942 - be aware WIRELESS IS BANNED at your location.
Comments (i.e. I will arrive on 08-Aug-2012, but need to have e-mail active by 01-Aug-2012 so I can start forwarding e-mail from my existing account before I leave here.
Or, If available, I'd like my Linux default shell to be tcsh.
Or, I don't know which file server I'm supposed to use, but I'm working with Drs. Smith and Jones.
Or, I also need a GUS account.
